Job Summary

Location: Streetwork Uptown & Lower East Side

Essential Job Functions: 

  • Provide intake and ongoing assessment for services for homeless youth dealing with Substance Use Disorders and Co-occurring mental disorder
  • Assess youth for eligibility for certain supportive housing, supportive income (SSIS/SSD, Medicaid, Food stamps, SNAP), healthcare, vocational/educational resources, sexual health (HIV/STI screening and hepatitis care)
  • Assist with application process for supportive and permanent housing, advocacy in obtaining housing vouchers and subsidies, escorts to appointments, obtaining identification.
  • Provide ongoing supportive case management to assist client in addressing challenges so as to sustain acquired resources
  • Coordination of services and advocacy to assist youth with referral to treatment for substance use and mental health care
  • Participate in case conferencing
  • Maintain and prepare case records for documentation
  • Participate in staff trainings, staff meetings and staff group process workshops
  • Participate in bi-weekly supervision.

Secondary Job Functions: 

  • Reconcile Petty Cash
  • Identify new resources appropriate for population
  • Coordinate services with other Streetwork staff and on-site legal, medical, and psychiatric service providers
  • Facilitate access to daily living needs (food, clothes, hygiene supplies, space to rest) when needed
  • Escort


  • Bachelor’s Degree in social services, creative arts, liberal studies or any related field and 3 years’ experience in direct service and/or Associate Degree with 5 years’ experience in direct service or High School Diploma and 8 years’ experience in director service.
  • At least 1 year working with the homeless population, youth, or harm reduction
  • Experience working in a social services setting, OR
  • Personal experience with street survival activities (homelessness, gangs, drug economy, sex industry) with three years separation from involvement in these activities. Deep understanding of the relationship between personal experience and professional standards.
  • Knowledge of NYC subsidized housing options and financial benefits
  • Basic understanding of computer and systems for managing case records

  • Dynamic energy, able to multi-task and negotiate multiple demands on attention
  • Non-judgmental approach and ability to empathize with others
  • Clear and concise verbal and written communication skill, excellent interpersonal and organizational skills
  • Genuine interest in working with disenfranchised youth and young adults
  • Knowledge of  sexual health issues facing youth communities
  • Bi-lingual Spanish (preferred, not required)
  • Understanding of harm reduction perspective
